Is this the form you are looking at? https://dl.dropboxusercontent.com/u/128 ... ple_v2.pdf.
Hello everyone, I am an EU national and I am about to apply for permament residence. For this purpose, I wanted to use the form linked above, as it is much simpler than the PR or than the standard EEA3 forms.

I am sure it has been asked again, although I was unable to find it, but how comes this simplified version of the form does not include the list of the departure from the UK/ return to the UK dates in the last five years? It used to be there in the old EEA3 form. Should I attach a separate sheet to the form?

Thank you a lot!

There have been several different versions of the form over the years. If interested you can find some of them in the Gov UK archives website.

If you happen to have spent a lot of time out of UK or made lots of trips you may list such absences in the 'additional info' section of the form (or on a separate sheet).

Thank you.

This website linked below suggests for instance to use the 06/2014 version, compiling only sections 1,7,8,9,10,11,12 if one is a EEA national without other family members:

https://www.freemovement.org.uk/how-to- ... plication/

I may have a deeper look and see how the forms do differ.

Its your choice - you could even apply by letter. However a form probably helps give the application some sort of structure though.

Don't forget to send to the correct and up-to-date address, enclosing current fee (and biometrics details for any non-EEA applicants).

Thanks, that's a very useful reminder.
