Can the EEA sponsor apply with family member for PR?

Hi everyone, I am a non EU national married to an EU national who is practising treaty rights in the UK for 7 years now.
We both want to apply for PR using the EEA(PR) form after being married for 5 years now.
1) The question is, can we apply at the same time using the same form?
2) If so, who would be the main applicant on the form.
3) Can my partner be both my sponsor in section 3 and an applicant at the same time.
