Refusal to issue a Permmanent reidence card

I am an EEA citizen leaving in the UK for the past 7 years, have been denied the Permanent residence card because I haven't provided:
1: a copy of my Italian health insurance for the year 2011-12
2 a letter or any other evidence that I have been working between April 2014 to October 2014.
Now I have all these documents do I have any ground for appeal? or do Ineed to send a new application
