Dutch citizen Applying for PR- arrival date and other Q's

Hello,
Firstly thank you for this helpful site.
I want to apply for my permanent residency card and then for citizenship. I have dual Dutch and Australian citizenship and have lived in U.K. since the late 1970's with my Dutch passport.

1. I arrived in UK around 1976/1977 with my Australian passport, then left to live in Netherlands for about a year where I obtained my Dutch passport, then came back to UK as a Dutch citizen. Unfortunately I don't have the exact dates anymore - my old passports were stolen in a house burglary.
Question 1. Should I estimate a first date of arrival for the form? And should this be the date as a Dutch citizen?

2. I have worked in UK in a mixture of roles - both as a PAYE worker (from 1979 with gaps) and also since 1989 as self-employed but at same time as being a Paye worker. Because the evidence required on the PR form for self-employment is so complicated I have decided to just use my Worker status(11 years continuously 2002 to 2013) as evidence and supply employer letters, P60s.
Question 2. Should I answer 'yes' to the question asking if I have ever been self employed, but say that I don't need to supply evidence because I'm applying as a Worker?

Question 3. Could you advise what other evidence I need to supply for the period 2002-2013?

Question 4. Do I need to supply evidence of residence from 2013 up to today as well?
