EHIC without start date?

Hey everyone,
I'm filling in the online application. I was a student between 2010 and 2012 and had CSI from my home country. As evidence I have an EHIC card that runs out 31/12/2012. Unfortunately it doesn't have a start date on it. Did anyone have success applying with an EHIC like that? Or do I need some sort of additional confirmation?
