EEA permanent residence card application

I am applying for a permanent residence card and I am unsure about to proceed.

I was issued a Residence Permit for a National of a Member State of the EEC in 2003, it expired in 2008 and I did not renew it.

When applying for a permanent residence card now, should I say that :
I’ve previously been issued with a document certifying permanent residence, permanent residence card, or equivalent document, and wish to replace that document because it’s been lost or stolen, or renew it because it’s expired.
or
I’ve lived in the UK for a continuous period of 5 years as an EEA national qualified person.

I have lived in the UK for more than 10 years.

Any help in clarifying this would be appreciated.

As you haven't got a PR document it's the second option you need to choose.

Many thanks for your reply.

I am filling in the online application, and the questions being asked go way past my 5 qualifying years. Is this is normal ? I would have thought I only had to answer to questions relevant to the qualifying periods .

If someone could clarify this for me that would be great.
