UK PR Requirements based on 5 years Retain rights

Deal All,
I got my Retain rights of residence in Jan 2013 after my divorce with my EEA national wife. Now I want to apply for Permanent residence in Jan 2018 after completion of my 5 years on retain right.
Can somebody please give advise on the following:

1) Documents required to submit for the PR based on my 5 years of retain rights? (No period of marriage included)
2) I checked online from the home office site there is a requirement of valid ID card /passport of the EEA national need to be submitted with the application. Will this be applied in my case or not? OR Is there any requirement of any documents required from my x-wife?

Your advise/help will be greatly appreciated.

Asking for yourself or another person as your previous topics indicate you applied for ILR on Set(O) under the UK immigration rules in 2012)??
