Could you please advise, I will submit application EEA2 for my wife and daughter (9 years old)
1- do I need to use online form or paper one
2- Can I use one form for both

It depends on your situation if you are self-sufficient I don't think you can, otherwise you can and I recommend using the European Passport Return Service as it is faster.
