EU supose to UK, her way to become British? Help

Hi guys!
I have dual citizenship, polish and british (british I receive in November 2017).
My wife is polish, we got married in september 2016.
She is in UK from 2013.
We are married and because I have british citizenship we decide she is going to become British as well.
I find out she has to be in uk for at least 3 years to become british, she need permanent residency and life in uk and language test...
So I have question;
We are going to fill EEA form for her to get her permanent residency and which section we have to fill in this form?
straight after she receive approval and a card she can apply for citizenship, and which section we have to fill on AN form?
