My wife forgot to mention my US and CANADA visa refusal while filling the EEA spouse visa Uk Online application?

my wife applied for EEA Spouse visa for me in Uk. while filling the online form she forgot to mention my US and CANADA visa refusal.i was thinking should I write an explanation letter and attach the refusals letter before submitting the documents to VFS Islamabad ? I have not submit my application to VFS Islamabad yet. I will submit it on Monday .please help me guys

hello everyone,
I hope everyone is good. I have a question if someone can answer me pleaseeee
I submit my documents with VFS Islamabad for EEA FAMILY PERMITS while submitting my documents the guy scanned my documents with digitalized scanner. the guy who scanned my documents returned my marriage certificate and my TB certificate plus my application form.
is it normal to return these documents ? I don't know if he put my documents in sequences , I am so worried.
please someone answer me I will be appreciated.
