Application form question - PR for EEA Citizen and NON-EEA family member

I am applying for both myself and my husband for permanent residency.
I am EU citizen and my husband has NON-EU family member permit expiring in 5th March 2019.
When applying, do I:
- use one application form for both of us, applying for myself as the main applicant then including husband as a family member (this seems more logical as I am applying for two documents, one for myself and one for him)
- or use application forms with him as a main applicant and me as his sponsor
What do I put in the below section:
“Your personal reference numbers (page 18)
Home office reference numbers. Include any registration certificate or residence permit numbers.”
When my husband applied for the NON-EEA family permit 5 years ago, he got back a letter from the home office which has the “ “our ref” and “case id” and his vignette in the passport has a number.
Do I put “Our ref” and the number from the passport numbers in? Should I make a note of which is which?
If I am the main applicant (my husband being a family member of the main applicant),
do I still need to fill in the details in section 11 (page 58)
“Complete this section if you’re applying for a document certifiying permanent residence or permanent residence card because you are (or were) the spouse, civil partner, or durable partner of the person named in section 2 of this form “your sponsor” “
This is what confuses me, as I am the main applicant and he is a family member, do I still fill in the details where it says “your sponsor’s name and other details?
When it comes to "fingerprints in previous immigration applications" (page 12)
Is there a way we can find out the exact date? My husband had his fingerprints taken some time in Oct/November 2009 but I can't find the exact date. The date of the letter inviting him to do so is October, but his biometric residence permit was issued in November 2009.
My husband can't remember the exact date as it was nearly 10 years ago, of course.
What would be the best way to go about putting in these details?
