European Family Permit information

Dear Guru,
One of my friend who is German citizen, living in UK since 2004 who want to sponsor his real brother, his wife and 5 children. I got following question.
1) Which application form to be used? i think it would be European Family Permit
2) Do they have to submit one application or each person have to submit separate application?
3) Is there any visa fee for this, when he went for application appointment, VFS asking £55 fee
4) What document needed?
