For the family permit application, who apply the EEA citizen or his spouse?
The spouse.
EEA citizens have automatic rights, so they don't need to do anything to prove their right to enter the UK. It is the spouse who usually needs additional documentation to be admitted in the country, so it is the spouse who needs to apply. The "sponsor" mentioned in the form would be in this case the EEA citizen.
