Lost BRP

Hi Team Members,
Am asking for a very good friend.
She lost her Biometric Residence Card(Permamnent Residence Card) which she had had since over 6yrs ago.
Now, she called the HO and was told to apply for a new BRP only to rejected after 4months.
She has also tried to convert it to Settled Status, still reject immediately.
Does anyone knows how to go about this or request for replacement please? Any link or website, gov.uk didn’t capture this information.
