How can non EU family memebers of dual British/EU citizens and who already have BRC permanent residence apply for settle

Hello! I am dual British/EU citizen and my wife is non EU citizen, who received her BRC permanent residence due to me exercing my treaty rights under Lounes case couple years ago. I believe my wife will still need to apply for settle status despite already holding permanent residence. Can she apply online or should it be papper application? Do we need to provide all evidence of me exercising my treaty rights since she arrived to UK or is it enough if we will provide Home office with my wife's BRC + evidence, that I continued to be qualified person/exercising my treaty rights after my wife received her BRC (evidence of residence and P60s, confirmation letter from NHS Trust, which I am working for this period of time). What else do I need to prove to make sure my wife will not be refused settle status? What address should we send her application form/evidence to? Is it true, that if we will not send evidence of relationships, she will be refused her right to appeal in case if they will not grand her settle status? Thank you for taking time reading my post!
