EEA Family Permit Query

Hi Everyone,
I'm a Belgium national and living in the UK for last one year. I'm applying for my sister and her kids visa under EEA Family Permit to call them in the UK. She's a single mum living with her three kids. My question is
Do I need to fill four separate applications, one for my sister and three for her kids or its going to be just one single application for my sister and mentioning three dependent in their?
