EEA3 & 4 Rejected, Need Baby Identity and residency evid

Hi
Our forms were returned on Saturday asking for 1 year old Baby passport and evidence of residency.
I have already sent the UK birth certificate and Czech nationality certificate but wasn't enough
For residency:
I am sending the Child health record book which includes immunisation and weight record which I think would suffice the residency criteria.
For identity
the baby name is on mother passport only. which I will mentioned now in covering letter with an extra copy.
Mother has Czech passport and baby is named on here passport.
My question is will the baby name in mother passport be enough for identity?
as it says in the guidance only Passport and Identity cards are acceptable as evidence.
