do I need sponsorship form

Hi,
I have a question. I am an EEA-national and I want to make an EEA-Family Permit application for my husband who lives in Pakistan.
My question is, do I need a sponsorship form? And if yes, where can I get that form? Do I need to certify that form by a sollicitor?
I have found 1 sponsorship form, but I am not sure if it's the good one.

Someone told me that I can also make a sponsorship letter myself. and certifying by a sollicitor isnt necessary.
So can anyone help me, becoz I am not sure.
