Which EEA Form should I use...???

Hello guys... I've been checking the forum for hours and couldn't find a answer for that.

I'm Brazilian married to a Italian and we lived here for nearly 5 years. I have a 5 years visa which will finish in August. I need to renew it but I can't find out which EEA Form i should use.
My friend who had the same situation last year said is the EEA1 because as we got the 5 dependent visa we are considered an European, but when I was reading the form it doesn't seen as i could use it.

And should I apply in my name or in her name adding me as a Family Member..??

Please I will be really thankful if someone can help me.

As OP is Brazilian (non-EU) he should apply using form EEA4 not EEA3 (Which is for EU nationals).

Correct!

When you have lived here for a continuous period of 5 years with the EEA or Swiss national (who must have been in employment, self-employment, studying or self-sufficient in the UK throughout the 5 years), you can apply for confirmation of your right to permanent residence in the UK.

You can apply for a permanent residence card by completing the EEA4 application form
