Comprehensive Sickness Insurance - for permenant residence?

Hi all

just a quick question about CSI - An EEA national came to the UK in 2004 as a young person (14 years old) and went straight into school. received her residence permit shortly after arrival. she now wishes to apply for permenant residence but says that she is unaware of whether she ever had insurance.

In the event that she does not - will her application be refused?

Was CSI a requirement in 2004? if not, can she still be penalised for studying for 5 years without it?

Has she been a student all the years since 2004? Did she apply in 2004 as a student?
If she did receive a residence document as a student in 2004, then according to the new instructions - annex B, she will not need to provide evidence for CSI.
