EEA4 Question!

Hi, I'm about to send my EEA4 application next week and had a question regarding the application itself, I have looked in many posts but could not find an answer to my question. Hope some one can help. Will be very grateful.
In section 6 page 12 it asks about the past 5 years of the EEA family member treaty rights.
I've been married for over 5 years (married in feb 2006, residence stamp ends in oct 2011) my wife had been in continuous employment in the UK since Jun 2005 but stopped working in Jun 2011, I believe according to some info i found here she has already acquired her PR.
The documents we are sending as prove of employment (EEA national)are her P60'S since 2006 until 2011 together with her P45 from her most recent job (leave date june 2011) . Will those be sufficient to support the application?
Now, in the form it asks for the relevant dates of the past 5 years of her treaty rights which we stated as June 2005 until June 2011. She has been off work since June 2011 until present. Do we have to note it in the application at all? or the dates mentioned will cover the application? At present she is taking some time off, and is not looking for a job. Should we mention it on the application? if so under what option?
Hope some one can share their opinion or knowledge related to my question.
