do I put the date I arrived on my work visa in 2009 or when I returned from a trip to Canada in 2011? Was in Canada for 12 days. Flew in to Dublin airport both times so no UK stamp.

I would take the most literal interpretation, which in your case would then be the date you returned from Canada last year. Or may be you have crossed the Irish border a few more times since then without even noticing?

Anyway, this question is totally irrelevant and I don't understand why it is on all EEA forms.

I interpret this to mean the first time one entered the country to begin residence.

May be wrong.

Put the last date you remember entering the UK if it was recent, if you don't know, state don't know or put that in covering letter. Don't make it up if you're not sure The actual date may be irrelevant to your application (there are so many cases, it's not worth listing them all here).

Thank you for all of your help. I have put the date that I last entered to UK. I have explained this in the cover letter as well. I sent my application off yesterday. I felt really nervous sending it off. hopefully it will only take 3-4 months to get my RC. fingers crossed!
