Visa vignette for applying driving license

Hi all! I am trying to apply for a provisional driving license and I have a few questions:
1. My visa is issued before 2015 so I do not have a BPR, do they still accept the visa vignette on my passport?
2. My visa vignette is in my old passport which is no longer valid and I have a new passport. It is completely fine for me to enter and leave the country with the visa in my old passport but would DVLA accept that?
3. If no. 2 is accepted, do I just include 2 passports in the post/
4. If the application is successful, I would like it to be delivered to my working address, is that possible?
