Visa for Wife from pakistan

Dear All,
I am applying for my Wife's visa and I am on Tier 1 General. I got extension in 2013 and my visa is valid till 2016 (i.e. 2+3 years). I am not sure which form I need to fill and what guidance will apply on me. I would really appreciate any help in this matter.

I have another question as well, My NIkkah was done 5 years ago and but Rukhsatee was done recently.

I was never in contact with my wife in that 5 years. After rukhsatee I am in contact with her. Does this situation matters in my visa application?
