ILR for Tier 1 General Dependant applying separately

Hi,
Hope you can help me too.
I have just got my ILR few days ago from Tier 1 General. My wife holds a Tier 1 General Dependant visa for the same time as I have (5 years). She need to apply for her ILR as well. (We should have applied together on SET(O), but we decided she should post her application after I got ILR.

What form will she use to apply? SET (O) or SET (M)?
I am waiting for my Biometrics card so she can post her application.

What documents would she need to include in her application.
If she is using SET(O), would I need to fill all the information Under Section 5 - Home and Finance, what would she fill in 5.2 and 5.3 as she is not working at the moment.
Would she need to show any earnings for Section 9A - Tier 1 (General) migrants for details of earning as she is a dependant and didn't earn anything.

How many months bank statement would I need to provide and do they have to me the immedaite month or would i provide the exacts ones I used for my ILR application showing period earning was claimed for.

Would the dependant need to show any of her personal account statement as well
