How to apply residence permit of a baby of 30 months family life visa

Hi All

I need urgent assistance. I would like to apply a residence permit for my 6 months old baby. When she was born our application was with the home office. We received our 30 months family life permit not long ago. I would like to apply for the baby but not sure which form to fill and the cost of application.
