Spouse visa extensio Application question

I am applying for my spouse visa extension in March . I am currently working in NHS as a doctor. I heard that NHS worker were exempted for IHS in visa applications. Can I ask for exemption in my spouse visa extension or its just for them who are on Tier 2 visas with NHS sponsorship.

You will still need to pay for IHS and you can claim it later.
