I would appreciate it if anyone could be able to assist with my confusion please

1- When is the safest to apply for FLR spouse extension visa without affecting the 5 years UK residency? Is it from 28 days before completing the 30 months since first day entered the UK? Or 28 before expiry day of first spouse visa?

2- Is B1 English test acceptable or it has to be A2?

3- As for accommodation; it is still not clear if we need to submit the same documents we previously sent in the first spouse visa like;
No obligation landlord letter? Or this time it is all about cohabitation proofs?
