Wife’s BRP has her old name. Her passport has her new name and also underneath it, “also known as: her old name”.

She wants to travel to her home country. Will this be a problem if she’s to return to the uk?

So her passport has her new name, which is her English name + my surname.

Underneath it is her old foreign name + family name, which is classed as “also known as”.

How would that be a problem? People change names all the time. That's why they have fingerprint for confirmatory identification. She can travel with a copy of her marriage cert for rest of mind.
No sin in failing, you just have to try and try again!
