Spouse Visa and Dependant Child Outside UK

Hi,
Im applying for a spouse visa for my wife. She and my 2 kids are in pakistan. My younger kid is uk born so i have applied for ROA for her and applying for spouse visa for my wife. Question is for the younger kid.. so far in the application, there were questions regarding the dependant kids in the application but im guessing those are not automatically added in the application and would need her application separately?
i have filled the form uptil the payment page.. would it ask to include the dependant child within the application after the payment? or give me an option to create an attached additional application for her? or does it have to be a completely separate application? same form?
Please advice.

It falls under AppendixFM child.. but now that i have filled out all the details the IHS fee is showing as zero.
am i doing something wrong? i dont want to submit the wrong application. I have gone thru this 3 times and each time its the same.. if anyone can shed some light on this? please
