I am confused which form i need for wife visa extension?
She was dependent via PBS and now i got ILR via 10 years long route,so for extension which form i need within UK?

As you were granted ILR under long residence, your wife will have to apply under the FLR(M) category. This will re-set her immigration clock to zero, putting her on a new 5 year path to settlement.
