Incountry switch HSMP - Spouse - Please Help!

My husband wants to do an in-country switch from his HSMP to a ILR spouse visa as I have ILR now. 2 questions:

1) Is the form FLR(M) the right form for him?
2) Are these the only documents that need to be send with the form?
2 Photos of him
1 Photo of me
Both our passports
Both our bank statements
Both our payslips
Marriage Certificate.

Any help would be highly appreciated!

All I want to know is if I need to show proof of our relationship or not on the form. As it is an in-country switch from HSMP to my spouse.
