help with MN1 - passport is already with UKBA

Hi Seniors,

I have got ILR this week and now preparing my daughters - born in UK Oct'10 MN1 form.

my initial application for Tier 1 General Extension was refused(no documents returned - including passport) meaning my daughter's dependant application was refused too. to which i have appealed to Ho, subsequently i have withdrawn my appeal with my wife's and I have submitted for ILR and my wife has submitted for FLR(M). however i am not sure what would have happend to appeal of my daughter - would it be deemed as withdrawn as it was with my applicaiton.

anyway - query here.
1. My Daughter's indian passport is still with HO - if i submit MN1 can i put a covering letter explaining and refereing to original application and subsequent appeal?
2. how do i check the status of her appeal - do i need to withdraw it seperately to obtain passport?
3. is her application fall under Section 1(3)?
4. should Section 3 (as none of the parents are british, only father got ILR) & Section 4(as she is under 10) of MN1 need to be kept blank?
5. how to arrange for the payment - there is no page for the payment details?

There is no need to have anything but for the Daughter's Birth Certificate and your BRP showing ILR for a s1(3) application. If the HO have her passport just state that on her MN1 application. Her immigration status is irrelevant.

also - can you answer around last 2 questions - seciton 3 & 4 of the form and fees.
