It can take many months and more than a year, especially due to her arriving as a tourist. It is not permitted to switch to any other visa category if in the UK as a visitor so the application will likely take a long time and possibly be refused.Also is 11 months completely crazy or is it the norm?
If your wife arrived as a tourist, she needs to pay 150% of the actual cost of the NHS services used while she was here as a tourist. If you want advice that badly, you should be contacting a solicitor instead of relying on a free forum. And to answer your question, you can't report your passport as lost when it actually hasn't been lost.
