Need Help How to file G-235A and I-130

Hi my name Charmaine and I need help on how to get a visa for my husband.
Well we know each other for year, and I travel back and forth. My parents going back to Vietnam to live so I'll be by myself. My husband and I decided to get marry so he can be with me. I found out I have to filled the application form I-130 and G-235A. There some parts I'm confuse if there anyway anyone can help me. Here is the part I'm confuse.

On form G-235A after filled the top parts which is my name.
On the parts that ask Family name and it have father or mother name Date and city Country of Birth ( If know ) do i put my name or my mother and my father name? Below it said husband or wife I need to put my husband name rite? IF anyone know about this please help me. Thanks you.
