OCI application queries

Hello All,

I am applying for an OCI for self (Adult) from the UK.

Could you please help me with a few queries.

1) Marriage Registration Date - do I need to enter marriage registration date or actual marriage date?

2) Place Of Issue * of UK passport - as no specific place of issue on UK passports, what should be entered here (HMPO ?)

3) Document upload section asks "Evidence of self or parents or grandparents or great grandparents" - Can I upload my indian passport here (cancelled after surrender)

4) In "Registration Form Part B" it asks about Name / Relationship of other family members who have applied / granted OCI card. - does relationship also need to mention in space available for name details e.g. name relationship details.

1) Marriage Registration Date - I read at few place it need to be Marriage Registration Date and not actual 1) Marriage Date

it will be great if someone please clarify.
