PSW Dependant visa application (case specific advice needed)

My case is follows:
I have had my nikah (religious marriage) which is considered legally and we have marriage certificate. We will have our wedding in April 2011. I want apply for her visa so that when we have the wedding reception she can come back with me.
Do we apply for the visa saying that we will be getting married in April or that we are already married?
We have had our engagement which could possibly be shown as wedding photos and we have met on other few occasions and have those photos as well. I have been in the UK for more than 12 months.
I would appreciate if you could answer the following questions:
a) Does she need an English language test?
b) Does she need a tuberculosis (TB) test?
c) Can I apply now?
d) Does she have to make an entry in to the UK within a certain time period after obtaining the visa?
e) Can calling cards along with emails be accepted? (Since we normally talk visa calling card, there is not much to say on emails, so they are very short and occasional. Do you recommend that we start building it up?)
I have been very confused with the documentation needed and people seem to have their own views. Despite looking at the ‘guides to supporting documents’ for the dependants it is not clear.

Some say that you need photos and emails and people’s visas have been rejected as they did not have them attached?
I am working full time, can possible obtain a rental agreement but the landlord would give me the proof of this ownership of the property. So what documents do we submit? I have prepared a list below with question marks against the ones I am not sure about and would appreciate your advice.
1. A letter from me (what key point to state?)
2. My passport (original or copy)?
3. My PSW visa card (original copy)?
4. VAF10 visa form
5. My 3 months bank statements (my balance £2000)
6. My 3 months pay slips
7. Our marriage certificate
8. Photographs
9. Emails
10. Calling cards?
11. Rental agreement? (How long do I have to have lived in the property for? I currently live in shared accommodation and only want to get a flat before leaving for my wedding?)
12. Her passport size photograph
Please advise me.

In addition, do I send all my originals to her and then she submits them to Gerry’s in Pakistan? (as that’s where she resides). Ideally I want to fill in the form myself and send it to her along with the documents?

I would like to add here that the above information has been written giving a future scenario to aviod complicating the situation. I have currently on Tier 4, but aim to switch to PSW by next month.
