filling citizenship application form

hi every one,can some one advice me on what to put on the AN form section 1.17,were it says to put your previous addresses for the last five years.

the first address i field in is my current address.I have field the FROM part but do not know what date to put on the TO part or should i leave it blank because i am still living there.if not what date should i used.

have u lived on different address during ur stay in uk or only one address

thanks for your reply.I have lived in two different addresses plus the one am living presently.on that section of the form i have started with my current address.

ya but ur current address should be on section 1, 1.16 and ur second last and third last on 1.17 hope u got it
