HSMP extension - FLR(IED) or FLR(HSM)?

HSMP extension - FLR(IED) or FLR(HSM)?

I am going to apply for HSMP extension on April 12th. As far as I know I should fill out FLR(HSM) form to apply for extension. Has it changed to form FLR(IED) for the applications effective from April 1st 2004? Can anyone confirm this please?

Ugur,

Yes you are correct, From 1st April FLR (HSM) will no longer be valid.

You have to complete FLR (IED) and there is a payment of £121 processing fee.

The documents no longer have to go to Doncaster. You have to send them to Walsall the payment processing centre

But I guess you would have to send the FLR HSM form too right ?
