Tier1 extension - which addetse to use? Please help!

Hi,
I will apply for a Tier1extension next week but I am not sure which address and dates I should provide in the application form. Here is my address movements on police registration form:
2009-2010 UK address I did not live here, just entered to the UK and registered to the police with this address.
May 2011- current UK address2 I live in this address since October 2010
but I registered this address only in may 2011

In between the first registry date (2009) and may 2011 I have UK address1 in the place registration form but actually I was not in the UK..
I came back to the UK in September and moved to my current address in October 2010. I registered to this address in may 2011.

If I provide the dates and addresses as in the police reg. form, then my bank statements can cause a problem as my bank statements have been sent to my current address since October 2010.

What should I do? Also should I explain them that I was not in the UK for a year but that doesn't exist on my police reg. form?

put the actual dates you lived there.

Thank you for the response but don't they cross check the address and the dates with the police registration form? Also do i need to put my non UK address in the form ( it does not appear on the police registration form).. Thank you so much
