Marriage visa: who fills it out?

My fiance (UK) and I (US) are working on our fiance visa with him as my sponsor. My questions are:

a) Should HE fill out the form as me, or should I fill out the form with him as my sponsor?

b) We are using his income, but there is nowhere in the application to indicate that, only a section asking for my work information, which is pretty measly. I'm worried that my $500 a month is going to get me denied.

c) I saw that there is an Appendix 2 form that can be filled out. I've seen older posts referring to it, but I'm not sure if that what we want for this application which all the new rule changes.

Applications are submitted by the person seeking entry clearance (the visa) but there is nothing to prevent the sponsor doing the actual typing if that is necessary. On the form, all the questions are asked directly to the applicant even when the answer is referring to the circumstances of the sponsor.

Of course there are questions about the sponsor income so I suggest you read more carefully the financial appendix.

When filling out the online form on the Visa4UK there are no appendices. I am assuming that we print off the pdf and fill it out by hand.

Does anyone have advice on this?
