There is no strict enforcement as what category must contain what specific documents rather all documents must be attempted to place metaphorically to the available categories. As long as all relevant documents are uploaded under whichever category, there shouldn't be any issue.

Though may include it but no requirement for Appendix 2 anymore as it is all incorporated into the online form.

Like hundreds of people since it came out on this forum that it is not mandatory, I applied without Appendix 2.

No, but you need a letter from your landlord stating that they have no objection to your wife living with you in the property.

The guidance in terms of accommodation is, no account shall be taken of a child under the age of one. so you are okay on that.
A studio flat is one bed roomed.
If you can afford it just pay for a property inspection report, i always say don't give ECO any chances to refuse you a Visa. Also note if you mention your wife is expectant, it will raise massive red flags.

Given the present languorous visa processing and the airline threshold within which a pregnant woman can board the flight might punctuate your plan.

There is no requirement to mention pregnancy. It is irrelevant to the spouse visa application.

Why are you trying to complicate things, pregnancy does not prove anything and like @CR001 said, it's irrelevant.
Note, the ECO is at liberty to exercise Means Test in making their decision on matters finance. In which if you talk of pregnancy there will be a child involved and they will need to be sure you can support the extra member. financial requirements will take a whole new dimension.
KISS. (Keep it simple and straight forward)

Also i suggest you include this in Family Category.
-sponsor divorce certificate
-chat logs
-photos.
and add Employment Contract on Employment category

Handful of pregnancy related letters can be included in subsisting relationship evidences.
