I have a few couple of questions and I am combining them here.

Well, you need to submit this - scan and upload with the other documents.
5. Under the documents checklist: It says “Bank statements or household bills are not suitable proof you have been living in the UK”. Please what can be used? It's just your passport(s) covering the residency period. This relates to EU citizens whose passports don't get stamped, or if you are submitting an expired passport.

This is a great question, and I am also not sure about the answer. I would think it won't hurt to include the withdrawn application as long as it is clear that it's withdrawn. As in... days? weeks? months? I have been trying to get even an acknowledgement from the Home Office after three weeks of requesting the withdrawal, and I have heard nothing. See this post.
