Which english test is that? When was it taken?

Many thanks for your help so far.
I have uploaded all the required documents in the portal and have only one doubt now.
In the mandatory documents list , is says the following two documents should be signed and uploaded.( consent form and declaration form)

1. Consent form for the Home Office to request
verification checks on (My name) for joint accounts or third party
support

2.Declaration signed to( My name) to confirm consent for the
Home Office to request verification checks

But I can see only the consent form could be downloaded and there is no as such any formats for declaration forms.
Could you please help me with ? Also in both the consent and declaration form request only my name is there and not my wife .. so wondering why ? did I do any mistake during application processing ?

Also as I mentioned earlier , my wife and son applying ILR together and I am the Sponser . Inside the consent form there is 3 sections.

Part 1 applicant
Part 2- Joint account holder
Part 3 : Sponser

So if understood correctly, part 1 should be signed by my wife and 2 ( me ) and 3 again my me as me being sponser?

Also Declration format ( which I dont find anywhere in the application link etc) .. is it two different declaration forms I need to submit .. one for my wife and other signed myself ?

For wife - Wife signs Part 1, you sign Part 2,3
For son - Part 1 Any Parent signs if he is a minor, otherwise he signs, you sign Part 2,3

But they asked to submit both consent and Declaration forms , whereas I could find only the consent form .

Thanks for all the help you extended so far in completing application and submitting/uploading documents.
One last question before I submit all the documents. When I sign the part 3 of the consent form, ( financial sponser) that I agree I would be sponser etc., which is fine. But in the consent form it is mentioned that I have provided information or documentation to support the application( meaning support financially etc)

I have uploaded my BANK statements as the proof ( just last months and 3 months before) ..
DO I need to upload any other documents other than bank statement ( like payslips, divident slips , my limited company certs , employment proof etc?)

appreciate your help again.

You do not need to provide any proof or documents for that. This is a generic consent form. Do not think too much about it.

Dear all
I have a query submitting originals to UKVCAS office .
I have appointment next week , submitting ILR for my wife and Son ( Tier 1 PBS dep).
Have uploaded all the required docs in the site.
On top of taking original Passports, BRP , do I need to take all original evidences I submitted/uploaded on Cohabitation ? ( around 55 docs Uploaded) ? Also do i need to take original marriage certs, english test etc?

Do we need to submit photocopies as well ? if so i can take copies at home itself , rather than in center where it should be expensive.

Just take the originals only. No need to take any print outs for soft copies. Nothing will be checked except passport and appointment confirmation. Take the soft copies in a USB.
No photocopies needed.

Yes I have submitted the docs yesterday . so now it should be with UKVI.
