Documents Required for FLR(M)

Can anyone let me know what documents of evidence are required for FLR (M) with in UK ?

Is it required for Settled person Passport to be send along with FLR application?I have gone through the guidence but havn't find much information.Can any one please let me know what documents are required for FLR?
