New Home Office Reference Number

I am making a new first time application for a Tier One - Entrepreneur - Self Employed - Visa. The application asks for a Home Office Reference Number. Where do I apply for this? I am applying from the US.

Question D1. Did the applicant obtain entry clearance or a visa before entering the UK?

I am OUTSIDE the UK and am making my Tier One application from the US. Does this question only apply to those making an application from INSIDE the UK?

Or are they asking if I ever had obtained any visa before entering the UK on other previous trips there?
