Naturalisation and AN form queries

I have a fixed penalty with 3 points and 60 pounds fine - SP50

In the application there are four sections, Can someone please let me know what to fill in that sections.
Nature of offence:
date sentenced:
sentence given:
country where sentenced:

You weren't sentenced. You were given a fixed penalty, so this doesn't apply. It's referring to someone who had an offence heard and the penalty given in court.

Doesn't make sense, to me at least.

If you are director of a company (X) that you are controlling shareholder of, you are employed by that company (X). Everyone else is a client of that company (X) and you provide services to clients on behalf of the company (X).

Sorry its my mistake to make it clear:
I work on a contract basis - I got the job through company B (Requirement agency) to work for a company A (Software company) and I am generating invoices from my own limited company to company B.
Hope it clears something.

For tax purposes your employer is your own company and it is your own company details that should be entered on to the form AN.

In that way the form AN will match up with your Self Assessment tax returns.
