Query on Tier 1 Extn

I am going for PEO appointment on 29th June at Solihull as Limited Company director. I would like to discuss the following queries for my extn.

1. My accountant has shown gross and net payments in his letter for the entire year. Do i need to ask him to include breakdown of monthly gross and net dividend and salary in his letter ?

2. What should i fill up in the Tier 1 dependant extn form for the L3 column ?

Kindly clarify these queries
