Partener settled in the UK

Hi All, need some advise:
I am British citizen and my wife is not from EEA. She has indefinite leave to remain granted in October 2010. We were planning to travel to other EU countries and wanted to know how she can apply for EEA family permit. I could not find the relevant information on UKBA web site as the information is more related to other EU countries but not including cases where the husband is British citizen and spouse is not EEA and has ILR.
Which form is the correct to be used when applying.
