Don't know, but recall spending 30 mins with new spouse completing a similar form before lunch in front of a shop load of school truents playing computer games in an internet cafe. To save time will my future wife be able to apply for her passport on her maiden name, then apply for the visa on that passport and then fly on her maiden name? On the other hand will it be better (although the process will take longer) to apply for her passport in her married name after marriage and then apply for the visa once she receives the passport?
